As our Cardiac Navigator RN at MercyOne, Coordinates and provides care for groups of high-risk individuals and/or populations in the inpatient and/or community setting. Assists other health care personnel in identifying health care needs. Interacts with patients and families and all levels of personnel, medical staff, community services, and insurance providers. Identifies and coordinates educational programs for patients and health care professionals, as well as the community.

You are very knowledgeable in all areas of your role but always eager to learn more. You keep impeccable records and knowledge of the current electronic medical record system. Confidentiality is of utmost importance to you. Most of all, you understand how vital your role is in the diagnosing and treating of our patients and you’re eager to help us succeed in making them better.
*Day-to-day overview:*
* In collaboration with multidisciplinary team, evaluates patient referrals to develop individualized plan of care across the continuum of care
* Acts as a liaison among the individual/family, community, provider and physician as well as other health care team members.
* Provides clinical leadership by being a preceptor for visiting professionals, students and new graduates.
* Ensures collection and input of risk stratification data. Reports results to the health care team and participates in interpretation of data and formulation of action plan.
* Is accountable for the implementation and communication of standards for patient care delivery appropriate to the specific patient population that meets professional and regulatory standards.
* Coordinates the health care team in development of clinical tools for optimal outcomes and reports findings.
*Qualifications*
* Current Iowa registered nurse (RN) license.
* Demonstrates competency within a specialized area of clinical practice with 4 years’ experience in that specialty.
* Proof of completion of Mandatory Reporter – Child and Dependent Adult Abuse training within three (3) months of hire.
* Basic Life Support certified within six (6) weeks of hire. Acceptable credentialing bodies and certifications include American Red Cross CPR/AED for the Professional Rescuer and American Heart Association Basic Life Support for Healthcare Providers.
* Demonstrates success in positions of leadership.
