At Atria, we are recruiting for a full-time and a part-time Health and Wellness Coach (HWC). This is an exciting opportunity to contribute to an innovative healthcare model focused on combining medical expertise with a wellness orientation. We are building an integrated care team to support patients and their self-efficacy in their health journey.

Specifically, you will:

  • Review patient health assessments and meet patients at their readiness of change
  • Hold in-person and virtual coaching sessions
  • Leverage motivational interview skills to support patients self-efficacy and progress
  • Document coaching sessions within the EHR
  • Work collaboratively with clinicians and providers as one integrated care team in service to the patient
  • Participate in care team meetings
  • Support developing a coach-approach environment with care team colleagues
  • Contribute to Atrias knowledgebase and learning materials on various health and wellness domains for patients and colleagues
  • Support continuous learning and development of self, team, patient and Atrias care model

Job Requirements:

  • Minimum education: Bachelors Degree in a related field
  • NBC-HWC credential; registered dietitian licensure a bonus
  • Minimum of two (2) years of HWC experience, with a minimum of 100 coaching sessions
  • Strong communication skills, verbal and written, with clients, patients and clinical team members
  • Computer literacy and proficiency with computer-based documentation systems
  • Open to feedback with a desire to continuously learn and grow
