The Health and Social Program Senior Director is a critical leader on the Community Strategy team that is responsible for defining Community & State’s program strategy for improving social risk factors and health equity within communities and for UnitedHealthcare members through the development and management of a portfolio of health and social programs that specifically address Social Determinants of Health. This work includes concept development, execution, program testing, operationalization, and measurement in markets where C&S holds a Medicaid managed care contract. This work is done in collaboration with national functional partners and health plans. The Senior Director will also be responsible for representing the health and social program portfolio within the enterprise to executive leaders to champion the strategy, garner buy-in for programs and position programs to support C&S’ business plan objectives.
The Senior Director will manage a dynamic team of health and social program experts and be responsible for the national footprint of C&S’s health and social programs such as Housing+Health, food is medicine/food insecurity, social isolation, and workforce development. The Senior Director will ultimately accountable for the end-to-end concept development, scaling, and return of investment of the services within the health and social program portfolio.

Primary Responsibilities:
- Develop, lead and champion the end-to-end health and social programming business strategy for Community & State including executive communication and reporting
- Work cross functionally and influence leaders to support the health and social programs and align efforts collectively to advance the program objectives (pilot, operations, program integrity) including collaborating with network, health care economics, finance, the office of the CMO, local market leaders, clinical operations, business development, marketing, and policy
- Manage and expand existing Housing+Health program portfolio:
- Align housing and health systems for housing insecure complex populations receiving Medicaid (e.g. Medicaid expansion, disabled, aged, individuals with complex behavioral health conditions and intellectually and developmentally disabled)
- Responsible for leading operations of existing programs and expanding footprint of programming across C&S’s 31 existing Medicaid markets
- Standardize and embed program operations within the markets and assessing performance (affordability, member impact, housing stabilization
- Normalize program capabilities and performance across all markets focusing on sustainability, outcomes and embedding of programs within health plan operations
- Define and execute a partnership strategy to formalize relationships with housing providers, housing support services, housing location services and clinical programs skilled at treating housing insecure and homeless patients.
- Define strategy to bring the Housing+Health program for all remaining C&S markets
- Lead the development of programs to address housing needs for aged, blind and disabled individuals within Medicaid waiver programs
- Leverage learnings and existing program infrastructure to scale Housing+Health programs to impact complex populations
- Execute a partnership strategy to formalize relationships with housing providers, housing support services and housing location services
- Represent Housing+Health program with external partners and identify strategic providers to support program performance
- Develop innovative strategies for building networks of non-traditional providers with the goal of aligning high performing, high quality providers to programs, execute partnerships and deploy network within markets
- Build data management/analytics approach to create meaningful insights using new data sources to identify members, better understand housing needs and couple housing data with health care data to inform program development. This includes cross functional collaboration with date management teams, technology and health care economics
- Inventory and work cross functionally with enterprise assets and teams to advance the Housing+Health strategic goals and improve member resource impact (e.g. UHC Catalyst, innovation teams, efforts for social determinants of health, etc.)
- Track external market to identify new funding opportunities, innovative capabilities and emerging models to addressing health and housing stabilization and use those insights to advance program objectives
- Identify evidence-based practices and seek innovative ways to adopt in existing programs and inform development opportunities (e.g. housing first, medical respite, money follows the person, tenancy supports, rapid re-housing)
- Develop, scale, and ensure return on investment of new health and social programs:
- Create disability employment program in various state markets, as well as develop partnerships with community-based organizations at the state, regional and national levels to provide a pathway to employment for candidates with a range of disabilities within the communities UHC serves
- Establish best-in-class social isolation programming for members and communities that supports resiliency and overcoming generational trauma
- Design and execute food is medicine model for Medicaid members in addition to broader food insecurity strategies for communities

Required Qualifications:
- Bachelor’s degree or higher
- 8+ years in progressively responsible positions in end-to-end program development/management within a matrix environment in managed care or related healthcare industry or public social programming
- 8+ years managing large cross-functional teams with direct and indirect reporting relationships
- 8+ years of experience in developing, managing and building innovative relationships with external customers and / or community partners
- 5+ years of P&L and budget management responsibility
- Experience negotiating and managing vendor contractors
- Experience coaching to drive performance, build relationships, establish credibility with individuals who do not directly report to position
