A healthcare network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff as their new Case Manager.

Responsibilities:

The Case Manager (RN) will:

• Schedule and review completed assessments
• Submit assessment tools and care plan documents to Manager of Clinical Services for review
• Educate members on Advanced Directives/Health Care Proxy and issues related to management disease specific conditions
• Complete assigned re-assessments and responds to changes in member’s condition/sentinel events
• Initiate interventions to coordinate care required to stabilize member’s condition
• Respond to requests for service and provide the authorizations within Medicare/Medicaid regulated time frames utilizing appropriate correspondence as per unit workflows
• Participate in and present cases at interdisciplinary care team meetings to discuss any issues identified during assessment process
• Provide needed assistance to I-SNP clinical team who are working with members residing in the Long Term Care Facilities and are a part of I-SNP Program
• Provide needed assessment, counselling and support to the I-SNP members/member’s family to optimize member’s care and facilitate ICT coordination
• Perform other duties, as needed

Qualifications:

• 2+ years of Clinical Nursing experience
• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• Experience in a Long Term Care and/or Community setting
• Experience working with the Geriatric population
• Managed Care and/or Home Care background
• Working knowledge of Medicare and Medicaid regulations
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• Bachelor's and/or Master's Degree in Nursing
• Prior experience in an Interdisciplinary Service Delivery environment
