Job Description
The Key Program is seeking a Service Coordinator for the Family Networks Program in Fall River, MA. The Service Coordinator will work directly with the Department of Children and Families (DCF), within the DCF regional office.
Qualifications: The Family Networks Service Coordinator requires a Bachelor’s (BA/BS) Degree in related human service/clinical field and one (1) year of case management or direct care experience. Bilingual/bicultural applicants are strongly encouraged to apply. All positions require applicants who are at least 21 years of age or older and have a car and valid driver’s license for work use.
Responsibilities: As the Family Networks Service Coordinator, you are responsible for the coordination and case management of services between families and providers to ensure the appropriate level of placement, to include :

- Conducts intakes, interviews and assesses clients in preparation for referral to the network.
- Coordinates and facilitates treatment plan review meetings, initial diagnostic conferences and discharge planning meetings.
- Ensures appropriate service implementation and case coordination.
- Works with providers to ensure that client documentation, billing and data entry is accurate and timely.
