Job Description

Psychiatric Rehabilitation Services Coordinator PRSC Job summary:

Provide resident with stable therapeutic relationship
Orient the resident to the facility
Review and assist the resident in understanding the treatment plan and program schedule
Prepare and assist the resident with active participation In the treatment plan review
Provide and/or coordinate the delivery of the psychiatric rehabilitation services programs
Monitor the resident in the areas of self-directed care and for overall compliance with the treatment plan
Completing assigned sections of the MDS, assigned CNA's, and additional components for the resident comprehensive assessment, including: psychosocial, history, skills assessment, discharge planning etc.
Re-evaluating the resident on a quarterly basis and documentation the re-evaluation
Leading groups and individual counseling sessions as assigned
Available to work flexible hours (evening, weekend, holidays)
Other case management duties as assigned

Psychiatric Rehabilitation Services Coordinator PRSC qualifications:

Bachelors or Masters degree in human service profession (i.e. social work, psychology, sociology, counseling psychology)
Demonstrated sensitivity, ability and skills in working with and understanding the needs of the residents, family members, staff members and personnel from community agencies
Demonstrated familiarity and knowledge of currents trends in the rehabilitation of the mentally ill
The ability to present concise, meaningful written reports and articulate the social/psychological needs of the residents
The ability to accept and utilize professional supervision, consultation and in-service training
The ability to handle confidential data with professional discretion
Experience conducting groups
Familiarity with paperwork pertaining to job (ie. MDS, care plans, progress notes)
