Senior Network Director, Senior Care Services

The Senior Network Director for Senior Care Service line is responsible for the overall strategic planning, business growth, marketing, patient experience, performance improvement and support of the provider contracting and incentive management in the service line. The Network Director builds and maintains successful relationships internally and externally with post-acute providers. Included are long term care, assisted living, and personal care, retirement communities as specialized agencies, Area office on Aging, AARP, United Way, and Alzheimer’s Association.

JOB DUTIES AND RESPONSIBILITIES:

ESSENTIAL FUNCTIONS:

Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.

Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.

Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).

Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.

Complies with Network and departmental policies regarding attendance and dress code. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

Facilitates the Network development of the service line through coordination, growth, strategic planning and marketing initiatives with appropriate entities.

Cultivated, coordinates, coaches and supports operations and leadership team in collaboration with Physician leadership.

Works collaboratively with Chief of Geriatrics to promote, enhance and evaluate the efficiency of day to day operations as well as service expansion and provider relations.

Partners with physician and post-acute care facility leadership to maintain and build clinical and operational relationships.

Facilitates and leads the coordination of Network initiatives that support or maximize services for the older adult. Including but not limited to Senior Surgery, NICHE, and Center for Positive Aging, Adult Day Center, Bundled Payment Initiatives, Home Visit services.

Develops, implements, supports ongoing clinical performance improvement and operational process improvements through the demonstration of quantifiable clinical and operational outcomes, safety management, and operational efficiency.

Develops performance improvement operational initiatives focusing on resource utilization, clinical standards, operational efficiency and implementation of new services.

Assists in the development of service line and provider scorecards for clinical and operational success.

Actively participates on Network committees to represent Senior Care vision and promote opportunities for service line and providers.

Oversees the practice management of the Senior Care Physicians assuring clinical and operational metrics are met.

Provides day to day oversight and continuously reviews financial indicators to offer recommendations to improve revenues or reduce expenses.

Functions as a liaison with other hospital and SLPG departments to support and promote Senior Care Services.

Provides a monthly report to senior management that captures quality, customer services, financial, growth and people elements. Recommends action plans to sustain or improve areas.

Continuously monitors regulatory /payor / service line trends to educate Network and seek new opportunities.

OTHER FUNCTIONS:

PHYSICAL AND SENSORY REQUIREMENTS:

Must be capable of sitting for up to 4 hours at a time, standing for up to 1 hour per day, 1 hour at a time, walking for up to 2 hours per day, 20 minutes at a time.. Must have fine motor skills as it relates to the operation of a computer or other office equipment. Hearing as it relates to normal telephone conversation and possible transcription. Vision as it relates to proofreading typed material and general vision and peripheral vision. Frequently uses hands for typing, filing and running equipment.

POTENTIAL ON-THE-JOB RISKS:

Employees who work in patient care areas where they may have potential exposure to patients with suspected or proven tuberculosis (TB) must have the ability to wear a particulate respirator and be fit-tested in compliance with the current recommendations from the CDC (enforced by OSHA) or must provide physician documentation as to the inability to wear a particulate respirator.

SPECIFIC PROTECTIVE EQUIPMENT AVAILABLE:

Adaptive equipment for keyboard modification is available.

MOST COMPLEX DUTY:

Support and maintain administrative functions in the Senior Care Service Line consistent with the vision, mission, and strategic plan of the Network Wide Service Line.

SUPERVISION (RECEIVED BY AND/OR GIVEN TO):

Supervises: Director for Senior Care Services /Staff Centers for Positive Aging.

Receives Supervision: Service Line Administrator/ Senior Care. Sub section Chief /Geriatrics

COMMUNICATIONS:

Interfaces with physicians, management team members, employees and members of the health care team. Excellent verbal and written communication skills are required.

ADDITIONAL REQUIREMENTS:

The ability to supervise, organize and evaluate programs, staff and clients. Ability to manage fiscal resources and assets. Knowledge base and understanding of the appropriate State and JCAHO regulations, procedures, and community services.

MINIMUM QUALIFICATIONS

EDUCATION:

Master’s Degree in Health Administration, Public Health, Nursing, and Social Work preferred.

TRAINING AND EXPERIENCE:

Previous experience in developing and leading a service line or major initiatives for several departments for senior services, long term care, and / or caregivers. Strong business sense as well as collaborative skills to identify and implement services across the network.

WORK SCHEDULE: Day shift with varying hours as needed to perform all essential functions based on the needs of the department.
