As a Community Health Worker, you will outreach to members over the phone to perform a health risk assessment, create an individualized care plan, order services and facilitate inter-disciplinary care team meetings.

This position will require active and unrestricted Nursing licensure in multiple US States. Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)

This is a telephonic role in a fast-paced care coordination environment. Must be able to work 8 hour shift 9:00am-6:00pm Monday - Friday

Primary Responsibilities:

  • Making outbound calls and taking inbound calls to assess members' current health status
  • Follows specific timeframes for member outreach, health risk assessments and facilitation of care team meetings with the member, caregivers, and other members of the health care team to coordinate the members plan of care.
  • Creates an individualized plan of care using SMART goal format
  • Identify and facilitate community resources for members as needed
  • Identifying gaps or barriers in treatment plans and move to solution
  • Providing patient education to assist with self-management
  • Interacting with primary care providers, pharmacists and other clinicians on a continual basis to coordinate the members care
  • Encouraging members to make healthy lifestyle changes
  • Documenting and tracking findings

Required Qualifications:

  • High School Diploma / GED (or higher)
  • 3+ years of direct clinical experience
  • 3+ years of experience in a hospital setting, acute care, direct care experience OR 3+ years experience as a telephonic Case Manager for an insurance company
  • VA resident
  • A current, unrestricted LPN license in the State of VA
  • Experience with outbound calls to members, Physician’s, Pharmacy, Providers or similar medical facility
  • Ability to create, copy, edit, send & save utilizing Microsoft Word, Excel & Outlook
  • Ability to enter data and clinical documentation at time of assessments and follow-ups

Preferred Qualifications:

  • Experience / background in managed care
  • Experience working with Medicaid or similar vulnerable populations who have chronic or complex medical and psychosocial needs

Telecommuting Requirements:

  • Reside within the state of Virginia
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Ability to keep all company sensitive documents secure (if applicable)
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Soft Skills:

  • Ability to work independently and but also lend clinical support as needed to team members as needed
