ESSENTIAL RESPONSIBILITIES
Responsibilities of the Home HealthLPN/Vocational Nurse include, but are not limited to, the following:
- Providesdirect patient care as defined in the State of Wisconsin Nurse Practice Act
- Implementsplan of care initiated by the registered nurse
- Providesaccurate and timely documentation consistent with the plan of care
- Assesses andprovides patient and family/caregiver education and information pertinent todiagnosis and plan of care
- Participatesin coordination of home health services, appropriately reporting the identifiedneeds for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to theregistered nurse and/or Clinical Supervisor
- Usesequipment and supplies effectively and efficiently
- Participatesin personal and professional growth and development
- Performsother duties as assigned by the registered nurse

QUALIFICATIONS
To perform the job successfully,an individual should demonstrate the following competencies to perform theessential functions of this position:
- Graduate of an accredited practical nurse or vocational nursing program
- Has three years nursing experience. Community/home health or medical/surgical experience is preferred
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of Illinois
- Complieswith accepted professional standards and practice
- Demonstratesgood verbal and written communication, and organization skills
- Mustbe a licensed driver with an automobile that is insured in accordance withstate and organization requirements and is in goodworking order
- Possessesand maintains current CPR certification
