SUMMARY:

The Licensed Practical/VocationalNurse is responsible for providing direct patient care under the supervision ofa registered nurse. ESSENTIAL RESPONSIBILITIES

Responsibilities of the Home HealthLPN/Vocational Nurse include, but are not limited to, the following:

• Providesdirect patient care as defined in the State of Wisconsin Nurse Practice Act
• Implementsplan of care initiated by the registered nurse
• Providesaccurate and timely documentation consistent with the plan of care
• Assesses andprovides patient and family/caregiver education and information pertinent todiagnosis and plan of care
• Participatesin coordination of home health services, appropriately reporting the identifiedneeds for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to theregistered nurse and/or Clinical Supervisor
• Usesequipment and supplies effectively and efficiently
• Participatesin personal and professional growth and development
• Performsother duties as assigned by the registered nurse

QUALIFICATIONS

To perform the job successfully,an individual should demonstrate the following competencies to perform theessential functions of this position:

• Graduate of an accredited practical nurse or vocational nursing program
• Has three years nursing experience. Community/home health or medical/surgical experience is preferred
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of Illinois
• Complieswith accepted professional standards and practice
• Demonstratesgood verbal and written communication, and organization skills
• Mustbe a licensed driver with an automobile that is insured in accordance withstate and organization requirements and is in goodworking order
• Possessesand maintains current CPR certification
