Responsibilities
The Care Coordination Manager is responsible for providing leadership and managing all processes/services within Care Coordination; assumes responsibility for implementing standards of practice, evidenced based medical guidelines, departmental policies and procedures, and maintaining and improving the quality of patient care delivered.
Assures staff and program compliance with established standards, practices, policies/procedures and regulatory requirements; responsible for overall standards of patient care, financial performance, human resource management and staff engagement, patient/customer satisfaction, appropriate reporting and for coordinating program functions with the operations of other hospital departments to assure quality services at a reasonable cost for patients; responsible for directing continuum of care practices that address the biological, developmental, psychosocial and clinical needs of the patient and/or family.
Creates an environment which enables the system to fulfill its mission and goals, fosters communication and collaborative practice with physicians and other departments.
Qualifications
Required Minimum Education:
- Bachelor's Degree. Will consider years of experience in case management in lieu of Bachelor’s or Master’s degree.
Preferred Education:
- Bachelor's Degree of an accredited school of Nursing (BSN).
Required Minimum Experience:
- A minimum of 3 years experience in the management of a multidisciplinary staff in the field of case management or quality and patient safety with at least 3-5 years of experience in a hospital environment
Required Minimum Certification:
- Current RN license in the State of Georgia, or LMSW.
- BLS
Required Minimum Skills:
- Strong interpersonal skills to facilitate team dynamics are necessary.
- Time management skills, leadership skills, prioritization skills and change management skills are necessary to focus on goal achievement for the system.
